Billing

Payment is due at the time services are rendered. Payment may be made by personal check, cash, or debit/credit card. 

Insurance companies I am in-network with include:  Humana Military, Forward Health, Badgercare, UHC, Molina

If I am not a provider with your insurance, I can provide you with reports and documentation for you to submit to your insurance carrier.  An itemized billing statement with all required diagnostic and procedural codes as well as service provider information is issued to the parent at the time of service. It is always a good idea to check with your insurance provider to determine if benefits are provided for private (i.e. out-of-network or non-participating) speech language pathology services.

Initial Evaluation 
Speech (Articulation) Only - $120
 Speech and/or Language - $200
Dyslexia Testing - $250


Individual Therapy Sessions 
30 minutes - $45
45 minutes - $60
60 minutes - $80

*All evaluations include evaluation and interview time, a written report and a follow-up meeting to go over results.
